Here are the Class I and II Personal Health and Medical Record forms. This form is required for Cubs attending camps,
Camporees or other programs. The Pack would like all Cubs to complete and return the Class I portion at the beginning of each school
year for the Pack's records. The Class III form is also available. This form is generally only needed for
adult Scouts (>40 yrs) attending camps or other events requiring a physical exam.
